Hospital Case Manager (RN)
Location: Winfield, IL (On-site)
Schedule: Full-Time, Monday–Friday (with occasional weekend flexibility)
Salary: $75,000 - $80,000 per year
About the Role
Are you an RN who thrives on solving the complex challenges of patient care? Community Physicians is looking for a dedicated Hospital Case Manager (RN) to join our Department of Transitions of Care.
In this vital role, you aren’t just processing paperwork; you are the primary architect of a patient’s recovery journey. You will act as the bridge between acute hospital care and post-acute settings, ensuring that our medically complex older adults transition safely, with dignity, and with the resources they need to avoid hospital readmission.
What You’ll Do (Key Responsibilities)
- Lead the Transition: Collaborate daily with physicians, therapists, and nursing staff to develop and execute individualized discharge plans for geriatric patients.
- Clinical Advocacy: Conduct comprehensive assessments to identify clinical, psychosocial, and financial barriers to a safe discharge.
- Resource Coordination: Arrange for essential post-acute services, including Skilled Nursing Facilities (SNF), Home Health, Rehabilitation, and Hospice.
- Documentation Excellence: Maintain precise, real-time documentation within the EMR to ensure compliance with federal, state, and payer regulations.
- Family Support: Provide counseling and resource education to families navigating the complexities of elder care and chronic disease management.
What We’re Looking For
- Education: BSN (Bachelor of Science in Nursing) is required.
- Licensure: Active, unrestricted RN license in the State of Illinois.
- Experience: 3–5 years of experience in hospital discharge planning, case management, or a related post-acute care field.
- Knowledge Base: Strong familiarity with Medicare/Medicaid guidelines, payer systems, and community resources in the Winfield/DuPage County area.
- Soft Skills: Exceptional critical thinking and the ability to remain calm and organized in a fast-paced hospital environment.
