Registered Nurse – Case Manager
Location: 501 Morris St, Charleston, WV 25301
Shift: Days | 5x8-hour shifts (8:00 AM – 4:30 PM)
Position Overview:
The RN Case Manager will coordinate discharge planning, care transitions, and patient advocacy in an acute care hospital setting. The role involves managing 35–40 patients per day, ensuring real-time documentation, and collaborating with interdisciplinary teams to optimize patient outcomes.
Requirements:
- Experience: Minimum 2 years of acute care experience
- Certifications: BLS (AHA required)
- Licensure: WV or Compact RN license (pending accepted)
- Travelers: First-time travelers accepted
- Weekend: Every other weekend required
- Floating: Within scope as needed
Unit Details:
- Dress: Business casual with white lab coat/jacket required on the floor
- Focus on Discharge Planning & Care Coordination
- MCG (Milliman Care Guidelines) experience preferred
- Experience with Risk Assessments and Social Determinants of Health (SDOH) a plus
- Charting System: Cerner
- Facility Type: Level I Trauma Center, Major Teaching Hospital (COTH member)
